Definitions
QUALIFYING RELATIVE The Qualifying Relative is the spouse or live-in partner. For single members the Qualifying Relative is the sole surviving parent. Partner is defined as a person, whether of the same or opposite sex, who lives with the member in a relationship. Single members with both or neither parents alive do not have a Qualifying Relative.
QUALIFYING CHILD A Qualifying Child is a member's child under the age of 18 years.
UPPER AGE LIMIT There is no upper age limit in membership categories. Age will not be a barrier to participation in the Benefits scheme.
Benefits Scheme Rules, Regulations and Requirements
1. There is an initial qualifying period of thirteen weeks from commencement of membership deductions, during which time claims cannot be paid or accrued.
2. Benefits expressed as being available for a '52 week period' are based on a rolling year system. That is to say, that during the 52 week period up to and including the date of treatment the amount claimed cannot exceed the amount specified in the table above.
3. The date of treatment will be used to calculate the amount to be refunded. This will be based on the details stated on your receipt. Delayed submission or payment will not increase the amount payable by BAWA Healthcare and Leisure.
4. In order for claims to be valid a current Benefits Claim Form must be completed and returned to BAWA with the necessary supporting documents. Forms are available online, by request or from on-site representatives.
5. For all claims that request refund for expenditure made a named receipt is required. The receipt must show the full names of the recipient and the practitioner, the type of treatment received and the date of treatment. Any handwritten receipts must bear the practitioners company stamp. Debit / credit card receipts are not sufficient.
6. BAWA Healthcare and Leisure reserves the right to check the details of any claim submitted and will take action against members considered attempting to or making fraudulent claims.
7. Claims will only be considered if the completed Benefits Claim Form is received by BAWA within 52 weeks of the treatment date.
8. Payment from the Home Help Benefit category can only be made for a proven medical necessity for home help. Claims should be accompanied with medical evidence such as a letter from the members G.P.
9. Convalescence is only available to members who have been absent from work due to medical necessity and have been a hospital inpatient or suffering from a terminal or stress related disorder. Forms will need to be completed by the members G.P.
10. BAWA reserves the right to alter the range and/or the category amounts available within the scheme. The current list of benefits is available online or on request from BAWA.
11. It is the responsibility of the member to inform BAWA Healthcare & Leisure of any changes to employment, address or personal details.
